How Does Empagliflozin Affect Glucose Handling?

SGLT2 is the main transporter responsible for returning filtered glucose from the kidney tubules to the bloodstream. Empagliflozin inhibits SGLT2, reduces renal glucose reabsorption, lowers the kidney threshold for glucose, and increases urinary glucose excretion.

Who Is Treated With Empagliflozin?

Empagliflozin improves glycemic control in adults and children age 10 and older with type 2 diabetes. In adults, it also reduces cardiovascular death and heart failure hospitalization in heart failure, reduces selected kidney and cardiovascular outcomes in chronic kidney disease at risk of progression, and reduces cardiovascular death in type 2 diabetes with established cardiovascular disease.

How Is Empagliflozin Dosed?

The recommended dose is 10 mg once daily in the morning with or without food. For additional glycemic control, the dose can increase to 25 mg once daily in patients who tolerate 10 mg. The 25 mg dose increase applies to glycemic control rather than the heart failure or chronic kidney disease indications.

What Serious Risks Are Linked to Empagliflozin?

Serious concerns include ketoacidosis, volume depletion, urosepsis, pyelonephritis, Fournier gangrene, genital fungal infections, hypoglycemia with selected diabetes therapies, lower-limb amputation, and hypersensitivity. Empagliflozin is withheld for at least three days before surgery or procedures involving prolonged fasting when possible.
