How Does Dicyclomine Reduce Intestinal Spasm?

Dicyclomine relieves gastrointestinal smooth muscle spasm through antimuscarinic activity at acetylcholine receptor sites and a direct effect on smooth muscle. These actions reduce intestinal muscle contraction, gastrointestinal secretions, and motility. Anticholinergic activity also accounts for reactions such as dry mouth, blurred vision, and increased heart rate.

Who Is Treated With Dicyclomine?

Dicyclomine is indicated for functional bowel or irritable bowel syndrome. Current labeling does not establish pediatric safety and efficacy and specifically contraindicates use in infants younger than 6 months. It is also contraindicated with glaucoma, myasthenia gravis, obstructive gastrointestinal disease, severe ulcerative colitis, reflux esophagitis, and obstructive uropathy.

How Is Dicyclomine Dosed?

Adult oral treatment starts at 20 mg four times daily. After one week, the dose can increase to 40 mg four times daily unless adverse reactions limit the increase. Treatment is stopped when adequate benefit is not achieved within two weeks or adverse reactions require doses below 80 mg daily.

What Safety Problems Need Attention With Dicyclomine?

Dicyclomine can cause dizziness, drowsiness, blurred vision, dry mouth, constipation, reduced sweating, increased heart rate, confusion, and other anticholinergic effects. Hot environments raise the risk of heat exhaustion or heat stroke due reduced sweating. Serious cardiovascular and central nervous system reactions have also been reported.
