How Does Dapagliflozin Affect Glucose Reabsorption?

SGLT2 in the proximal kidney tubules reabsorbs most filtered glucose. Dapagliflozin inhibits SGLT2, reducing glucose reabsorption and increasing urinary glucose excretion. This lowers blood glucose in type 2 diabetes. Its heart failure and kidney benefits extend beyond glucose lowering, though their full mechanisms are not defined in the label.

Who Is Treated With Dapagliflozin?

Dapagliflozin is used with diet and exercise for type 2 diabetes in adults and patients age 10 and older. In adults, it also reduces selected heart failure outcomes and reduces the risk of kidney decline, end-stage kidney disease, cardiovascular death, and heart failure hospitalization in chronic kidney disease at risk of progression.

How Is Dapagliflozin Dosed?

For glycemic control in type 2 diabetes, treatment starts at 5 mg once daily and can increase to 10 mg once daily. Adult heart failure, chronic kidney disease, and cardiovascular risk-reduction indications use 10 mg once daily. Kidney function and volume status are assessed before treatment, and kidney thresholds differ by indication.

What Serious Risks Are Linked to Dapagliflozin?

Serious risks include ketoacidosis, volume depletion, acute kidney injury, serious urinary infections, genital fungal infections, Fournier gangrene, and low blood glucose when used with insulin or insulin secretagogues. Treatment is withheld before major surgery or prolonged fasting when clinically appropriate. It is not recommended for glycemic control in type 1 diabetes.
