How Does Clopidogrel Reduce Platelet Aggregation?

Clopidogrel must be converted by CYP enzymes, principally CYP2C19, to an active metabolite. That metabolite binds irreversibly to platelet P2Y12 ADP receptors and blocks downstream platelet activation and aggregation. The effect persists for the life of the affected platelet.

How Are Clopidogrel Tablets Dosed?

Premature discontinuation raises the risk of cardiovascular events.

What Serious Risks Are Linked to Clopidogrel?

Clopidogrel carries a boxed warning for reduced antiplatelet activity in patients with two loss-of-function CYP2C19 alleles. Bleeding is the most common serious adverse reaction. Other concerns include thrombotic thrombocytopenic purpura and hypersensitivity.
