Cimzia is a prescription tumor necrosis factor blocker containing certolizumab pegol. It is given by subcutaneous injection. Current U.S. indications include Crohn disease, rheumatoid arthritis, polyarticular juvenile idiopathic arthritis, psoriatic arthritis, ankylosing spondylitis, non-radiographic axial spondyloarthritis, and moderate to severe plaque psoriasis under age and disease-specific criteria.

How Does Cimzia Block TNF Activity?

Certolizumab pegol binds human tumor necrosis factor alpha and neutralizes its biological activity. TNF alpha is an inflammatory signaling protein involved in immune responses and tissue inflammation. Blocking TNF activity reduces inflammatory signaling involved in the diseases treated with Cimzia.

Who Is Treated With Cimzia?

Cimzia treats adults with moderately to severely active Crohn disease after inadequate response to conventional therapy, rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, non-radiographic axial spondyloarthritis with objective inflammation, or moderate to severe plaque psoriasis. It also treats active polyarticular juvenile idiopathic arthritis in patients age 2 and older.

How Are Cimzia Injections Dosed?

Dosing depends on the condition. Several adult indications start with 400 mg at weeks 0, 2, and 4, followed by 200 mg every other week or 400 mg every four weeks. Crohn disease uses 400 mg every four weeks after the starting doses when response occurs. Pediatric polyarticular juvenile idiopathic arthritis uses weight-based dosing.

What Serious Risks Are Linked to Cimzia?

Cimzia carries boxed warnings for serious infections and malignancy. Tuberculosis, invasive fungal infections, and other opportunistic infections have occurred, so tuberculosis screening is completed before treatment and repeated during therapy when indicated. Other concerns include hepatitis B reactivation, heart failure, hypersensitivity, demyelinating disorders, blood-cell abnormalities, and serious immune reactions.
