How Does Cephalexin Kill Bacteria?

Cephalexin is bactericidal and inhibits bacterial cell-wall synthesis. Disrupting cell-wall formation weakens susceptible bacteria and leads to cell death. Its antibacterial activity covers selected gram-positive and gram-negative organisms, but resistant organisms such as methicillin-resistant staphylococci and Pseudomonas species are not susceptible.

Which Infections Are Treated With Cephalexin?

Cephalexin treats selected respiratory tract infections, otitis media, skin and skin structure infections, bone infections, and genitourinary tract infections caused by susceptible bacteria. It should be used only when a bacterial infection is proven or strongly suspected to reduce unnecessary antibiotic exposure and drug resistance.

How Is Cephalexin Dosed?

Adults and patients age 15 and older commonly take 250 mg every six hours or 500 mg every 12 hours for 7 to 14 days. More severe infections can require doses up to 4 g daily in divided doses. Pediatric dosing is weight based, and kidney impairment can require lower doses or longer dosing intervals.

What Serious Risks Are Linked to Cephalexin?

Serious risks include severe allergic reactions, Clostridioides difficile-associated diarrhea, seizures, altered blood clotting, and overgrowth of nonsusceptible organisms. Kidney impairment raises the risk of drug accumulation and toxicity. Cephalexin is contraindicated in people with known hypersensitivity to cephalexin or other cephalosporin antibiotics.
