Carotid artery stenting is a catheter-based procedure used to widen a narrowed carotid artery and support the artery with a metal mesh stent. Why Is Carotid Artery Stenting Performed?

Carotid artery stenting can be considered for selected patients with clinically significant carotid stenosis when revascularization is expected to reduce stroke risk. Patient age, symptoms, anatomy, prior neck surgery or radiation, medical conditions, and procedural risk influence the choice between stenting and endarterectomy. Current guidelines favor carotid endarterectomy over transfemoral stenting for many standard-risk symptomatic patients. Stenting remains valuable when surgical anatomy or comorbidity makes another approach preferable.

How Is Carotid Artery Stenting Performed?

A catheter is introduced through an artery, commonly from the groin or another vascular access site, and guided to the carotid artery using angiography. An embolic-protection system is commonly used to reduce the chance that plaque debris will travel to the brain. The narrowed area can be dilated with a balloon before or after a self-expanding stent is placed across the stenosis. Antiplatelet therapy is used around the procedure to reduce stent-related clot formation.

What Is Recovery Like After Carotid Artery Stenting?

Patients are monitored after the procedure for neurological changes, blood pressure abnormalities, heart-rate changes, and bleeding at the access site. Temporary low blood pressure or slow heart rate can occur because the carotid sinus is stimulated during ballooning or stent placement. The access site and distal circulation are checked while activity increases gradually. Follow-up commonly includes antiplatelet therapy and ultrasound surveillance to evaluate blood flow through the treated artery.

What Are the Risks and Complications?

Stroke or transient ischemic attack is the major procedural concern because plaque or clot can embolize to the brain. Other risks include death, bleeding, arterial injury, access-site hematoma, abnormal heart rate, low blood pressure, contrast reaction, and kidney injury. The stent can later develop restenosis from recurrent narrowing within or near the treated segment. Compared with carotid endarterectomy, transfemoral stenting has a higher periprocedural stroke risk in several patient groups, while endarterectomy has different surgical risks.

Is carotid artery stenting the same as carotid endarterectomy?

No. Stenting opens the artery from inside using a catheter and mesh stent, while endarterectomy uses open neck surgery to remove plaque directly.

Are blood thinners needed after carotid artery stenting?
