Budesonide and formoterol fumarate is a prescription inhaled combination of a corticosteroid and a long-acting beta2-adrenergic agonist. It is used for asthma in patients age 6 and older and for maintenance treatment of airflow obstruction and reduction of exacerbations in chronic obstructive pulmonary disease. It is not indicated for rapid relief of acute bronchospasm.

How Do Budesonide and Formoterol Work Together?

Budesonide binds glucocorticoid receptors and reduces inflammatory activity in the airways. Formoterol stimulates beta2-adrenergic receptors on airway smooth muscle, increasing cyclic AMP and relaxing bronchial muscle. The combination pairs anti-inflammatory corticosteroid activity with long-acting bronchodilation for ongoing asthma or chronic obstructive pulmonary disease treatment.

Who Is Treated With Budesonide and Formoterol?

The inhaler is approved for asthma in patients age 6 and older. It is also used for maintenance treatment of airflow obstruction and reduction of exacerbations in chronic obstructive pulmonary disease, including chronic bronchitis or emphysema. The 160 mcg with 4.5 mcg strength is the strength used for chronic obstructive pulmonary disease.

How Is Budesonide and Formoterol Dosed?

Patients age 12 and older with asthma use two inhalations of 80 mcg with 4.5 mcg or 160 mcg with 4.5 mcg twice daily. Children age 6 to under 12 use two inhalations of 80 mcg with 4.5 mcg twice daily. Chronic obstructive pulmonary disease uses two inhalations of 160 mcg with 4.5 mcg twice daily.

What Safety Problems Need Attention With Budesonide and Formoterol?

Risks include oral candidiasis, pneumonia in chronic obstructive pulmonary disease, adrenal suppression, reduced bone mineral density, slowed growth in children, cataracts, glaucoma, paradoxical bronchospasm, cardiovascular effects, low potassium, and high blood glucose. Patients rinse the mouth with water without swallowing after each dose. A separate rescue inhaler is used for sudden breathing symptoms.
