How Does Brexpiprazole Act at Dopamine and Serotonin Receptors?

The exact therapeutic mechanism of brexpiprazole is unknown. Its activity is thought to involve partial agonism at dopamine D2 and serotonin 5-HT1A receptors together with antagonism at serotonin 5-HT2A receptors. Brexpiprazole also interacts with other dopamine, serotonin, and adrenergic receptors, which contributes to its pharmacologic profile.

Which Conditions Are Treated With Brexpiprazole?

Brexpiprazole is used with an antidepressant for major depressive disorder in adults, as treatment for schizophrenia in adults and adolescents age 13 and older, and for agitation associated with dementia due to Alzheimer disease. It is not indicated as an as-needed treatment for Alzheimer-related agitation.

How Are Brexpiprazole Tablets Dosed?

For adult major depressive disorder, treatment starts at 0.5 mg or 1 mg once daily and targets 2 mg daily, with a 3 mg maximum. Adult schizophrenia starts at 1 mg daily and targets 2 mg to 4 mg daily. Alzheimer-related agitation starts at 0.5 mg daily and increases to a 2 mg target dose.

What Serious Risks Are Linked to Brexpiprazole?

Brexpiprazole carries boxed warnings for increased mortality in elderly patients with dementia-related psychosis and suicidal thoughts and behaviors with antidepressant treatment in younger people. Other risks include neuroleptic malignant syndrome, tardive dyskinesia, metabolic changes, compulsive behaviors, low blood pressure, falls, low white blood cell counts, seizures, and swallowing difficulty.
