How Does Botox Reduce Muscle or Gland Activity?

OnabotulinumtoxinA enters motor or autonomic nerve terminals and blocks acetylcholine release by cleaving SNAP-25, a protein involved in neurotransmitter release. In muscle, this produces temporary local reduction in activity. Intradermal injection reduces sweat gland activity, while bladder injections reduce acetylcholine signaling involved in detrusor contraction.

Which Medical Conditions Are Treated With Botox?

Botox has labeled uses for adult overactive bladder, neurologic detrusor overactivity in adults and selected children, chronic migraine prevention in adults, spasticity from age 2, adult cervical dystonia, severe axillary hyperhidrosis in adults, and blepharospasm or strabismus from age 12. Dose and injection sites differ across these conditions.

How Are Botox Doses Given?

Botox is reconstituted before injection and given into specific muscles, the bladder wall, or the skin according to the indication. Adult dosing should not exceed a total of 400 Units during a three-month interval. Pediatric dosing should not exceed the lesser of 10 Units per kg or 340 Units during the same interval.

What Serious Risks Are Linked to Botox?

Botox carries a boxed warning for distant spread of toxin effects beyond the injection site. Symptoms can include generalized muscle weakness, double vision, drooping eyelids, swallowing difficulty, speech problems, urinary incontinence, and breathing difficulty. Swallowing and breathing complications can become fatal. Other risks differ according to the injection site and treated condition.
