Biceps tendon repair is surgery that reattaches a torn biceps tendon to bone to restore muscle function. The term most commonly refers to repair of a distal biceps rupture at the elbow, where the tendon is reattached to the radial tuberosity. Repair can restore elbow flexion and forearm supination strength that is lost after a complete distal rupture. Why Is Biceps Tendon Repair Performed?

Distal biceps repair is commonly considered for active patients with a complete rupture who want to restore flexion and supination strength. Acute repair is generally easier before the tendon retracts and scars into surrounding tissues. Nonsurgical treatment can be appropriate for selected patients who accept some lasting loss of strength or have medical reasons to avoid surgery. The decision depends on injury location, time since rupture, activity demands, tendon retraction, arm dominance, and overall health.

How Is Biceps Tendon Repair Performed?

The surgeon identifies the torn distal tendon through a single-incision or double-incision approach and prepares the radial tuberosity for reattachment. High-strength sutures are placed through the tendon and secured to the radius with a cortical button, suture anchors, an interference screw, bone tunnels, or a combination of fixation methods. The repair is tensioned so the tendon sits securely against bone while nearby nerves and blood vessels are protected. Chronic retracted tears can require graft reconstruction when the tendon cannot reach the radius without excessive tension.

What Is Recovery Like After Biceps Tendon Repair?

The elbow is protected after surgery while the tendon heals back to bone. Motion is introduced gradually, but lifting and resisted elbow flexion or forearm supination are restricted during early healing. Physical therapy progresses range of motion first and then strengthening in stages. Return to heavy lifting, manual work, or sports commonly takes several months and depends on strength, motion, symptoms, and repair healing.

What Are the Risks and Complications?

Possible complications include infection, stiffness, rerupture, weakness, heterotopic ossification, and loss of forearm rotation. Sensory changes from irritation of the lateral antebrachial cutaneous nerve are among the more common nerve-related complications after distal repair. Injury to the posterior interosseous nerve is less common but can cause weakness of finger or wrist extension. Radioulnar synostosis, hardware problems, or failed fixation can occasionally require additional treatment or surgery.
