How Does Bendeka Damage Cancer Cell DNA?

Bendamustine forms reactive alkyl groups that bind to DNA and create interstrand DNA crosslinks. These abnormal links interfere with normal DNA structure and cellular processes and can lead to cell death. Bendamustine has activity against both resting and dividing cells, although its exact anticancer mechanism is not fully defined.

Which Blood Cancers Are Treated With Bendeka?

Bendeka is approved for chronic lymphocytic leukemia. It is also approved for indolent B-cell non-Hodgkin lymphoma that progressed during or within six months after treatment with rituximab or a rituximab-containing regimen. Its comparative efficacy against first-line chronic lymphocytic leukemia treatments other than chlorambucil has not been established.

How Are Bendeka Infusions Scheduled?

For chronic lymphocytic leukemia, the labeled dose is 100 mg per square meter by intravenous infusion over 10 minutes on days 1 and 2 of a 28-day cycle for up to six cycles. For indolent B-cell non-Hodgkin lymphoma, the dose is 120 mg per square meter on days 1 and 2 of a 21-day cycle for up to eight cycles.

What Serious Reactions Are Linked to Bendeka?

Bendeka can cause severe bone marrow suppression, serious infections, progressive multifocal leukoencephalopathy, infusion reactions, anaphylaxis, tumor lysis syndrome, severe skin reactions, liver injury, secondary cancers, and fetal harm. Blood counts are checked frequently during treatment. Liver tests and the infusion site also require monitoring during therapy.
