How Do the Two Ingredients in Auvelity Work?

Dextromethorphan acts as an uncompetitive NMDA receptor antagonist and a sigma-1 receptor agonist. Bupropion inhibits CYP2D6, which raises dextromethorphan blood concentrations, and has noradrenergic and dopaminergic activity. The exact mechanisms responsible for Auvelity treatment effects in major depressive disorder and agitation associated with Alzheimer dementia are not fully known.

Which Conditions Are Treated With Auvelity?

Auvelity is approved for major depressive disorder in adults and for agitation associated with dementia due to Alzheimer disease. The agitation indication uses scheduled treatment rather than dosing only when agitation occurs. Before treatment, clinicians assess blood pressure, bipolar disorder history, and use of other medicines containing bupropion or dextromethorphan.

How Is Auvelity Dosed?

For major depressive disorder, treatment starts with 45 mg of dextromethorphan and 105 mg of bupropion once each morning, then increases on day 4 to the same dose twice daily at least eight hours apart. Alzheimer-related agitation starts at 30 mg and 105 mg once daily, with staged increases on days 8 and 15 based on tolerability.

What Major Risks Come With Auvelity?

Auvelity carries a boxed warning for suicidal thoughts and behaviors in younger people taking antidepressants. Other serious concerns include seizures, high blood pressure, mania or hypomania, psychosis, angle-closure glaucoma, dizziness, serotonin syndrome, and low blood sodium. Monoamine oxidase inhibitors, seizure disorders, and certain eating disorders are among its contraindications.
