How Does Aristada Become Active Aripiprazole?

After intramuscular injection, aripiprazole lauroxil is converted through enzyme-mediated hydrolysis into aripiprazole. Aripiprazole activity is thought to involve partial agonist effects at dopamine D2 and serotonin 5-HT1A receptors and antagonist effects at serotonin 5-HT2A receptors. The extended-release formulation maintains exposure between injections.

Who Receives Aristada?

Aristada is approved for treatment of schizophrenia in adults. People who have not taken aripiprazole first establish tolerability with oral aripiprazole. Aristada is not approved for dementia-related psychosis. Dose selection is based partly on the oral aripiprazole dose that previously stabilized the patient.

How Is Aristada Started and Scheduled?

Aristada is given by a healthcare professional. Initiation uses either one 675 mg Aristada Initio injection plus one 30 mg oral aripiprazole dose with the first Aristada injection, or 21 consecutive days of oral aripiprazole with the first injection. Aristada doses include 441 mg monthly, 662 mg monthly, 882 mg monthly or every six weeks, and 1064 mg every two months.

What Serious Risks Are Associated With Aristada?

Aristada carries a boxed warning for increased mortality in elderly patients with dementia-related psychosis. Other serious risks include neuroleptic malignant syndrome, tardive dyskinesia, metabolic changes, compulsive behaviors, low blood pressure, falls, low white blood cell counts, seizures, and impaired judgment or movement. Weight, blood glucose, lipids, and movement symptoms need clinical monitoring.
