How Does Arikayce Act Against MAC Bacteria?

Amikacin is an aminoglycoside antibacterial that binds to the bacterial 30S ribosomal subunit and disrupts protein synthesis. Arikayce encloses amikacin in liposomes for delivery to the lungs by inhalation. The formulation exposes affected airways to amikacin while systemic blood concentrations remain lower than those seen with intravenous amikacin dosing.

Who Receives Arikayce for MAC Lung Disease?

Arikayce is reserved for adults with refractory Mycobacterium avium complex lung disease who have limited or no alternative treatment options. The labeled population has failed to achieve negative sputum cultures after at least six consecutive months of a multidrug background regimen. Arikayce is not recommended for non-refractory MAC lung disease.

How Is Arikayce Inhaled?

The recommended dose is one 590 mg per 8.4 mL vial inhaled once daily through the Lamira Nebulizer System. The vial is brought to room temperature and shaken before use. Arikayce is for oral inhalation only and is not injected. If a dose is missed, the next dose is taken the following day without doubling.

Which Safety Risks Are Linked to Arikayce?

Arikayce carries a boxed warning for increased respiratory adverse reactions, including hypersensitivity pneumonitis, coughing up blood, bronchospasm, and worsening underlying lung disease. Other serious risks include anaphylaxis, hearing or balance toxicity, kidney toxicity, neuromuscular blockade, and fetal harm. Aminoglycoside hypersensitivity is a contraindication.
