What Is Apokyn?

Apokyn is a prescription subcutaneous injection containing apomorphine hydrochloride, a non-ergoline dopamine agonist. It is approved for acute, intermittent treatment of hypomobility or off episodes in people with advanced Parkinson disease. These episodes include end-of-dose wearing off and unpredictable on and off periods. Apokyn is used as needed rather than on a fixed daily injection schedule.

How Does Apokyn Act on Dopamine Receptors?

Apomorphine stimulates dopamine receptors in the brain. Its exact Parkinson mechanism is not fully known, but its clinical action is thought to involve stimulation of postsynaptic dopamine D2-type receptors in the caudate-putamen. This receptor activity can improve movement during off episodes when regular Parkinson medicines are no longer controlling symptoms.

When Is Apokyn Used During Parkinson Treatment?

Apokyn is used for acute, intermittent off episodes in advanced Parkinson disease. It is studied as an adjunct to other Parkinson medicines rather than a replacement for the full treatment regimen. Doses are given when an off episode occurs, with at least two hours between injections.

How Is the First Apokyn Dose Given?

The first test dose is given under medical supervision while blood pressure and pulse are checked lying down and standing. The starting dose is 1 mg to 2 mg under the skin. Doses are adjusted according to response and tolerability up to 6 mg per injection. Mild or moderate kidney impairment uses a 1 mg starting dose.

Which Safety Issues Need Attention With Apokyn?

Apokyn can cause nausea, vomiting, sleepiness, sudden sleep episodes, low blood pressure, fainting, falls, hallucinations, dyskinesia, hemolytic anemia, impulse-control problems, coronary events, and QT prolongation. Intravenous use is unsafe. Apokyn is contraindicated with 5-HT3 antagonists such as ondansetron due reports of profound low blood pressure and loss of consciousness.
