How Does Apixaban Reduce Blood Clot Formation?

Apixaban selectively inhibits factor Xa, an enzyme needed for thrombin generation and clot formation. It acts on free and clot-bound factor Xa and does not require antithrombin III for activity. By reducing factor Xa activity, apixaban decreases thrombin generation and the formation of fibrin-rich blood clots.

What Major Risks Come With Apixaban?

Apixaban carries boxed warnings about increased thrombotic risk after premature discontinuation and spinal or epidural hematoma around neuraxial procedures. Bleeding is the main adverse effect and can be serious or fatal. Strong P-glycoprotein and CYP3A4 inhibitors or inducers can alter apixaban exposure, so medication interactions need careful review.
