What Is an ECMO Machine Used For?

An ECMO machine is used when severe heart or lung failure does not improve enough with standard treatments. It may be used for severe respiratory failure, certain heart failure emergencies, cardiac arrest support, bridge to transplant, or recovery after major heart or lung injury. ECMO gives the body time while the underlying problem is treated or while a longer-term plan is made. It is not a cure and is reserved for carefully selected patients.

How an ECMO Machine Works

Large tubes called cannulas are placed into major blood vessels. Blood flows from the patient into the ECMO circuit, where a pump moves it through an oxygenator. The oxygenator adds oxygen and removes carbon dioxide, acting somewhat like an artificial lung. The blood is then warmed if needed and returned to the patient.

Types of ECMO Support

Veno-venous ECMO supports the lungs by removing blood from a vein and returning oxygenated blood to a vein. Veno-arterial ECMO supports both circulation and oxygenation by returning blood to an artery. The type chosen depends on whether the main problem is lung failure, heart failure, or both. Cannula placement and machine settings are managed by an ECMO specialist team.

Risks and Monitoring

ECMO carries serious risks, including bleeding, blood clots, infection, stroke, limb circulation problems, hemolysis, kidney injury, or equipment complications. Patients usually need blood thinners, which can increase bleeding risk. The care team monitors oxygenation, blood flow, clotting labs, cannula sites, organ function, and machine alarms continuously. Sudden bleeding, neurologic changes, loss of pulses, infection signs, or circuit alarms require urgent attention.
