What Are Aminoglycosides Used For?

Aminoglycosides are used for selected severe infections caused primarily by aerobic gram-negative bacteria. They can be prescribed for bloodstream infections, complicated urinary infections, abdominal infections, and infections involving resistant organisms. Certain aminoglycosides are used for tuberculosis, plague, tularemia, or specific parasitic infections. They are sometimes combined with another antibiotic to broaden coverage or improve activity against selected bacteria.

How Do Aminoglycosides Work?

Aminoglycosides enter susceptible bacteria and bind mainly to the 30S portion of the bacterial ribosome. This disrupts the reading of genetic instructions and causes the production of abnormal proteins. The resulting damage interferes with the bacterial cell membrane and leads to cell death. Bacteria can become resistant through drug-modifying enzymes, changes to the ribosomal target, reduced entry, or increased drug removal.

How Are Aminoglycosides Given and Monitored?

Systemic aminoglycosides are usually given through a vein or into a muscle because most are poorly absorbed from the digestive tract. Some formulations are used in the eyes, ears, lungs, skin, or intestine for specific purposes. Clinicians adjust the dose according to body weight, kidney function, infection severity, and the medicine being used. Blood drug levels, kidney tests, fluid status, and hearing or balance symptoms can require monitoring during treatment.

What Are the Side Effects and Warnings?

The main serious risks are nephrotoxicity, ototoxicity, and neuromuscular blockade. Kidney injury is often detected through changes in creatinine or urine output, while ear damage can cause hearing loss, ringing in the ears, dizziness, or balance problems. Risk increases with prolonged treatment, high exposure, kidney impairment, older age, dehydration, or other medicines that affect the kidneys or ears. Patients should promptly report reduced urination, hearing changes, severe weakness, or breathing difficulty.

Are aminoglycosides broad-spectrum antibiotics?

Aminoglycosides act against many aerobic gram-negative bacteria and selected gram-positive bacteria, but they do not cover every organism. Their usefulness depends on the specific drug, infection, resistance pattern, and whether another antibiotic is given with them.

Why are most aminoglycosides not taken by mouth?

Most aminoglycosides are poorly absorbed through the gastrointestinal tract, so oral doses do not produce reliable blood levels for systemic infections. Oral or locally delivered forms are still used for certain targeted purposes.

Can aminoglycosides cause permanent hearing loss?
