An aldosterone antagonist is a medicine that blocks the effects of aldosterone at the mineralocorticoid receptor. These medicines are also called mineralocorticoid receptor antagonists. Blocking the receptor reduces sodium retention and limits potassium loss while also affecting harmful cardiac and kidney remodeling. Examples include spironolactone, eplerenone, and finerenone.

What Are Aldosterone Antagonists Used For?

Aldosterone antagonists are used in selected patients with heart failure, high blood pressure, resistant hypertension, or primary aldosteronism. Spironolactone can also treat edema related to certain heart, liver, or kidney conditions. Finerenone is used for selected patients with chronic kidney disease associated with type 2 diabetes. The specific indication, expected benefit, and dose differ among products.

How Do Aldosterone Antagonists Work?

Aldosterone normally activates mineralocorticoid receptors and promotes sodium and water retention while increasing potassium excretion. Antagonists block this signaling in the kidneys and other tissues. The resulting effects can reduce fluid retention, lower blood pressure, conserve potassium, and limit inflammation or fibrosis in the heart and kidneys. Their clinical benefit is not explained by diuresis alone.

What Types of Aldosterone Antagonists Are There?

Spironolactone and eplerenone are steroidal mineralocorticoid receptor antagonists. Spironolactone also interacts with androgen and progesterone receptors, which contributes to some endocrine side effects. Eplerenone is more selective for the mineralocorticoid receptor. Finerenone is a nonsteroidal antagonist with different pharmacologic and approved-use characteristics.

What Are the Side Effects and Monitoring Needs?

High potassium is a major risk and can cause dangerous heart-rhythm problems when severe. Kidney function can worsen, particularly in susceptible patients or when interacting medicines are used. Spironolactone can cause breast tenderness, breast enlargement, menstrual changes, or sexual side effects. Potassium, kidney function, blood pressure, other medicines, and dietary potassium sources require review and monitoring.

Are aldosterone antagonists potassium-sparing diuretics?

Spironolactone and eplerenone have potassium-sparing diuretic effects because they reduce sodium retention without promoting potassium loss. Finerenone is classified primarily as a nonsteroidal mineralocorticoid receptor antagonist.

Can aldosterone antagonists raise potassium levels?

Yes. Hyperkalemia is one of their most important risks. Blood potassium and kidney function are checked before and during treatment.

Are spironolactone and eplerenone the same?

No. Both block mineralocorticoid receptors, but they differ in selectivity, interactions, approved uses, dosing, and endocrine side effects.

Can aldosterone antagonists be taken with potassium supplements?
