What Is Alprolix?

Alprolix is a recombinant coagulation factor IX product used in adults and children with hemophilia B, also called congenital factor IX deficiency. It contains coagulation factor IX linked to the Fc portion of human immunoglobulin G1. Alprolix is given by intravenous injection after reconstitution and is used for bleeding episodes, surgery-related bleeding management, and routine prophylaxis.

What Is Alprolix?

How Does Alprolix Help Blood Clot?

People with hemophilia B have deficient or defective factor IX activity, which disrupts normal blood clot formation. Alprolix replaces factor IX and raises circulating factor IX activity. The Fc fusion portion extends the time the replacement protein remains in circulation, which permits longer intervals between prophylactic doses than some shorter-acting factor IX products.

When Is Alprolix Used in Hemophilia B?

Alprolix is approved for adults and children with hemophilia B for on-demand treatment and control of bleeding episodes, perioperative management of bleeding, and routine prophylaxis to reduce bleeding frequency. It is not indicated for induction of immune tolerance in patients with hemophilia B.

How Is Alprolix Given for Routine Prophylaxis?

Alprolix is given by intravenous injection after the powder is mixed with its supplied diluent. Adults and adolescents age 12 and older start with 50 IU/kg once weekly or 100 IU/kg every 10 days. Children younger than 12 start with 60 IU/kg once weekly. Dosing is adjusted according to clinical response.

Which Complications Are Associated With Alprolix?

Serious concerns include allergic reactions and anaphylaxis, development of neutralizing factor IX antibodies, and thromboembolic complications. Patients whose factor IX levels do not rise as expected or whose bleeding remains uncontrolled are evaluated for inhibitors. Nephrotic syndrome has been reported during attempted immune tolerance induction with factor IX products in certain patients.
