How Does Ajovy Block Migraine Signaling?

Fremanezumab-vfrm binds to the CGRP ligand and blocks it from attaching to its receptor. CGRP participates in migraine biology and pain signaling. Reducing CGRP activity lowers signaling associated with migraine attacks. Ajovy targets the CGRP molecule rather than the receptor and is used for prevention rather than treatment of an individual migraine attack after symptoms begin.

Who Uses Ajovy for Migraine Prevention?

Ajovy is approved for migraine prevention in adults. It is also approved for preventive treatment of episodic migraine in patients age 6 to 17 who weigh 45 kg or more. The current label does not approve Ajovy for pediatric patients below 45 kg since an appropriate strength presentation is not available.

What Dosing Schedules Are Used for Ajovy?

Adults receive 225 mg under the skin once monthly or 675 mg every three months, given as three consecutive 225 mg injections. Patients age 6 to 17 who weigh at least 45 kg receive 225 mg once monthly. Ajovy is injected into the abdomen, thigh, or upper arm. Trained patients age 13 and older or caregivers can administer selected doses.

Which Safety Issues Need Attention During Ajovy Treatment?

Ajovy is contraindicated in people with serious hypersensitivity to fremanezumab-vfrm or its ingredients. Current warnings cover hypersensitivity reactions, constipation with serious complications, new or worsening high blood pressure, and new or worsening Raynaud's phenomenon. Injection-site reactions are the most commonly reported adverse reactions. Severe constipation, breathing trouble, swelling, or painful color changes in fingers or toes need medical assessment.
