What Are Macrolide Antibiotics Used For?

How Do Macrolide Antibiotics Work?

Macrolides bind to the 50S subunit of the bacterial ribosome. This interferes with the movement and assembly needed to produce bacterial proteins. Resistance can develop through changes to the ribosomal target, drug-removing pumps, or enzymes that alter the antibiotic.

Food instructions differ among products, so patients should follow the prescription label rather than applying one rule to the entire class. What Are the Side Effects and Precautions?

Common effects include nausea, abdominal pain, diarrhea, and vomiting. Some macrolides can prolong the QT interval and increase the risk of an abnormal heart rhythm, especially in susceptible patients or with interacting medicines. Liver injury, hearing changes, severe allergy, and Clostridioides difficile infection can also occur. Clarithromycin and erythromycin have more clinically significant drug interactions than azithromycin because of differences in enzyme inhibition.
