What Are Gabapentinoids Used For?

Gabapentinoids are used as adjunctive treatment for certain partial-onset seizures and for selected types of nerve pain. Approved uses vary by drug and product and can include postherpetic neuralgia, diabetic peripheral neuropathy, spinal cord injury pain, fibromyalgia, or restless legs syndrome. They are also prescribed off label for some conditions, although evidence is not equally strong for every pain problem. The specific diagnosis, kidney function, other medicines, and risk of sedation affect treatment selection.

How Do Gabapentinoids Work?

Gabapentinoids bind to alpha-2-delta auxiliary subunits associated with voltage-gated calcium channels. This action changes calcium-channel trafficking and can reduce the release of excitatory neurotransmitters in overactive nerve pathways. The result can reduce abnormal electrical activity or neuropathic pain signaling. Their clinical effects are not produced by directly stimulating gamma-aminobutyric acid receptors.

How Are Gabapentinoids Taken?

Gabapentinoids are available in immediate-release and selected extended-release oral formulations. Doses are commonly started low and increased gradually according to response and tolerability. The kidneys remove much of the drug, so reduced kidney function can require a lower dose or longer dosing interval. Treatment should be tapered rather than stopped suddenly when withdrawal symptoms or seizure worsening are a concern.

What Are the Side Effects and Warnings?

Common effects include dizziness, sleepiness, blurred vision, impaired coordination, swelling, and weight gain. Serious breathing problems can occur, particularly when a gabapentinoid is combined with opioids or other central nervous system depressants or used by someone with respiratory risk factors. Misuse, physical dependence, and withdrawal symptoms have also been reported. Severe sedation, confusion, bluish lips, or slow, shallow, or difficult breathing requires urgent medical attention.

Are gabapentinoids opioids?

No. Gabapentinoids are not opioids and act through different molecular targets. Combining them with opioids can still increase sedation and respiratory risk.

Are gabapentinoids the same as gamma-aminobutyric acid?
