A fibrinolytic agent is a medicine that promotes breakdown of fibrin within a blood clot. These medicines are also called thrombolytic or clot-dissolving drugs. What Are Fibrinolytic Agents Used For?

Fibrinolytic agents are used in selected time-sensitive emergencies in which rapid clot dissolution can restore blood flow. Indications can include acute ischemic stroke, ST-segment elevation myocardial infarction, massive or high-risk pulmonary embolism, and selected blocked vascular catheters. The approved indication differs among individual drugs and formulations. Mechanical clot removal or another reperfusion strategy can be preferred when it is available and appropriate.

How Do Fibrinolytic Agents Work?

Tissue plasminogen activator-type medicines convert plasminogen into plasmin, especially near fibrin-containing clots. Plasmin breaks fibrin strands that provide structural support to the thrombus. Clot dissolution can reopen an obstructed blood vessel and restore oxygen delivery to tissue. The same mechanism can disrupt protective clots elsewhere and cause serious bleeding.

How Are Fibrinolytic Agents Given?

Systemic fibrinolytics are administered intravenously using drug-specific bolus or infusion regimens. Doses can depend on the indication, body weight, symptom timing, and other treatment factors. Emergency teams confirm eligibility through clinical evaluation, imaging, laboratory information, and review of recent procedures or bleeding. Blood pressure, neurological status, heart rhythm, oxygenation, and signs of hemorrhage require close monitoring.

What Are the Bleeding Risks and Contraindications?

Major bleeding, including intracranial hemorrhage, is the most serious complication. Active internal bleeding, certain recent surgeries or injuries, prior intracranial conditions, severe uncontrolled hypertension, and other factors can make treatment unsafe. Contraindications and precautions differ by indication because the expected benefit and urgency are not the same. Sudden headache, neurological decline, low blood pressure, or visible bleeding during treatment requires immediate evaluation.

Is alteplase a fibrinolytic agent?

Yes. Alteplase is a recombinant tissue plasminogen activator that converts plasminogen to plasmin and promotes fibrin breakdown.

Can fibrinolytic agents cause brain bleeding?

Do fibrinolytic agents work for every blood clot?
