Why Is a Duodenal Switch Procedure Performed?

BPD-DS is used to treat severe obesity and obesity-related conditions when the expected benefits outweigh the substantial nutritional and surgical risks. The operation can produce large and durable weight loss and strong improvement in type 2 diabetes and other metabolic conditions. It is often considered for patients with a very high body mass index or when another bariatric operation has not produced adequate results. How Is a Duodenal Switch Procedure Performed?

The operation is commonly performed laparoscopically or robotically under general anesthesia. The surgeon first creates a sleeve-shaped stomach and divides the duodenum beyond the pylorus. A distal segment of small intestine is connected to the duodenum so food bypasses a large portion of the small bowel. A separate biliopancreatic limb carries bile and pancreatic enzymes until it joins the food stream near the lower small intestine, creating a relatively short common channel for digestion and absorption.

What Is Recovery Like After a Duodenal Switch?

Patients begin with a staged diet that progresses from liquids to soft foods and then to a long-term bariatric eating plan. Protein intake, hydration, physical activity, wound healing, and bowel function are monitored during recovery. Lifelong vitamin and mineral supplementation and regular laboratory testing are required because the procedure substantially reduces nutrient absorption. Bowel movements can become more frequent, loose, or strong-smelling, particularly when dietary fat intake is high.

What Are the Risks and Complications?

Early surgical risks include bleeding, infection, blood clots, leakage from a staple line or intestinal connection, bowel obstruction, and complications related to anesthesia. Long-term problems can include protein-calorie malnutrition and deficiencies of iron, calcium, vitamin D, vitamin A, vitamin E, vitamin K, and other micronutrients. Gallstones, kidney stones, bone loss, diarrhea, foul-smelling stool, and excessive weight loss can also occur.
