What Are Carbapenems Used For?

Carbapenems can treat selected severe abdominal, urinary tract, lung, skin, bone, joint, bloodstream, and central nervous system infections. They are particularly useful against many bacteria that produce extended-spectrum beta-lactamases or other enzymes that disable common antibiotics. The antibacterial coverage differs among individual carbapenems. Culture and susceptibility results help clinicians determine whether a carbapenem is needed and which one is appropriate.

How Do Carbapenems Work?

Carbapenems bind to penicillin-binding proteins that bacteria use to build and maintain their cell walls. This blocks peptidoglycan cross-linking, weakens the wall, and causes susceptible bacteria to die. Their chemical structure protects them from breakdown by many common beta-lactamases, but not all resistance enzymes. Carbapenemase production, altered targets, reduced membrane entry, and efflux pumps can make bacteria resistant.

How Are Carbapenems Given?

Carbapenems used for serious systemic infections are usually given through a vein, although administration options differ by product and location. The dose and schedule depend on the infection, kidney function, body size, and specific medicine. Some regimens use prolonged infusions to keep drug concentrations above the level needed to inhibit the bacteria. Treatment can be narrowed to a more targeted antibiotic when test results and the patient's condition allow it.

What Are the Side Effects and Precautions?

Common side effects include nausea, diarrhea, headache, rash, and reactions at the injection site. Serious risks include severe allergy, Clostridioides difficile infection, seizures, blood-cell changes, and liver or kidney test abnormalities. Carbapenems can sharply lower valproic acid levels and increase the risk of breakthrough seizures. Careful prescribing helps slow the spread of carbapenem-resistant bacteria and preserve these antibiotics for infections that need them.

Are carbapenems related to penicillin?

Yes. Carbapenems and penicillins are separate groups within the beta-lactam antibiotic family. People with a history of a severe beta-lactam reaction need an individualized allergy assessment before treatment.

Are carbapenems available as tablets?

Carbapenems commonly used for serious systemic infections are generally administered by injection. Availability of specific products and formulations can differ among countries.

Do carbapenems treat infections caused by resistant bacteria?
