What Are Bisphosphonates Used For?

Bisphosphonates are commonly used to treat or prevent osteoporosis in patients at increased fracture risk. They are also used for Paget disease of bone, high calcium levels caused by cancer, and selected bone complications from cancer. The specific medicine, route, and treatment duration depend on the condition and risk level. Adequate calcium and vitamin D status are considered as part of treatment planning.

How Do Bisphosphonates Work?

Bisphosphonates attach to hydroxyapatite at sites where bone is being remodeled. Osteoclasts absorb the drug when they break down the treated bone surface. Nitrogen-containing bisphosphonates inhibit an enzyme needed for normal osteoclast function, while older non-nitrogen drugs disrupt osteoclast activity through other mechanisms. The overall result is reduced bone resorption and slower bone turnover.

How Are Bisphosphonates Taken?

Bisphosphonates are available as oral tablets or liquids and as intravenous infusions or injections. Many oral products must be taken after an overnight fast with plain water, followed by remaining upright and avoiding food or other medicines for a specified period. These steps reduce esophageal irritation and improve absorption. Intravenous products follow different schedules and can require kidney-function, calcium, and vitamin D assessment.

What Are the Side Effects and Precautions?

Oral bisphosphonates can cause heartburn, esophageal irritation, abdominal discomfort, and difficulty swallowing. Intravenous doses can cause temporary fever, muscle aches, or flu-like symptoms, especially after the first infusion. Rare but serious risks include osteonecrosis of the jaw, atypical femur fracture, severe hypocalcemia, kidney injury, and eye inflammation. Treatment duration and the possibility of a monitored drug holiday are reviewed according to fracture risk and the specific product.
