What Is The Procedure For Direct Inspection?

Direct inspection of the bulbar conjunctiva is typically performed using focused light, such as a penlight or a slit lamp, to assess its clarity, color, and vascular pattern. Normally, the bulbar conjunctiva appears shiny and transparent, allowing the underlying white sclera to be clearly visible, with only fine, sparse blood vessels visible.

To inspect the majority of the bulbar conjunctiva, the patient is asked to look in different directions while the examiner gently holds the eyelids open. For example, asking the patient to look nasally (toward the nose) exposes the temporal (outer) bulbar conjunctiva, and looking upward exposes the inferior (lower) bulbar conjunctiva. This technique stretches the loose, flexible membrane, making any abnormalities, like growths or dilated blood vessels, easier to detect.

How To Examine The Hidden Portions?

The bulbar conjunctiva is continuous with the palpebral conjunctiva at the conjunctival fornix, a deep recess or cul-de-sac. These areas (the superior or upper and inferior or lower fornices) are not visible during simple direct inspection. To inspect the inferior fornix, the examiner gently pulls the lower eyelid downward while the patient looks upward. This maneuver pulls the tissue downward, exposing the recess.

Examining the superior fornix is more challenging and requires a technique called upper eyelid eversion. This involves using a cotton swab or similar tool to stabilize the eyelid and flip it upward and outward while the patient looks down. Eversion is necessary to check for foreign bodies embedded under the lid or to inspect for signs of follicular or papillary inflammation that can be confined to the palpebral and forniceal areas.

What Abnormalities Should Be Noted?

During inspection, an examiner notes any deviation from the normal transparent and quiet appearance. Signs of inflammation, such as redness (hyperemia) and swelling (chemosis), are key indicators of conjunctivitis. The pattern of vascular dilation can also be informative, such as finding a bright red, uniform patch indicating a subconjunctival hemorrhage.

The examiner also checks the underlying sclera for discoloration. The presence of yellowing (icterus), which indicates high bilirubin levels (jaundice), is most easily seen against the white sclera through the bulbar conjunctiva. Growths like pingueculae (yellowish bumps near the cornea) or a pterygium (a triangular, encroaching growth) are noted, as these are typically located on the bulbar conjunctiva.
